Medical Director at EmblemHealth Health@Work

Sheila Louise Butler MD, MPH, FACOEM

Dr. Butler is the Medical Director of EmblemHealth Health@Work (formerly the Occupational and Environmental Health Center of Eastern New York). She is responsible for the daily operation of this occupational health clinic, one of the only occupational health network clinics in New York State.

Dr. Karl Auerbach Headshot

Dr. Karl Auerbach

Physician

Dr. Auerbach is a Staff Physician at EmblemHealth Health@Work. His primary activity is seeing patients, including complex workers’ compensation cases, surveillance evaluations, disability and fitness-to-work evaluations, World Trade Center cases, and veterans examinations, among a wide variety of occupational medical evaluations. Dr. Auerbach is a board certified occupational physician. He initially trained as an engineer and moved into medicine after a stint as a meteorologist in the U.S. Air Force. He attended the University of Rochester where he got his MD and MPH. He completed a residency in Internal Medicine and Occupational Medicine. Since then, Dr. Auerbach has practiced clinical occupational medicine for over 45 years in Rochester, NY, Philadelphia, PA, and now in Albany, NY. He enjoys skiing and sailing and spending time with his daughter.

Dr. Collina D. Cooke, LMSW

Social Worker

Dr. Cooke is the social worker and case manager at EmblemHealth Health@Work. In her role, she provides counseling services, case management, crisis intervention services, and support to patients experiencing work-related illness or injury. She is a three-time graduate of the University at Albany (SUNY). Dr. Cooke obtained a Bachelor of Science (magna cum laude) in 2014, a Master of Social Work in 2015, and a Doctorate in Social Work in 2021. She went on to become a Licensed Master Social Worker (LMSW) in New York State. Before joining the Health@Work team, Dr. Cooke worked with underrepresented college students, homeless families, and individuals with substance use and mental health disorders. Dr. Cooke was born and raised in Brooklyn, NY, and has lived in the Albany area for over 10 years. She is passionate about exploring holistic ways to decrease the physical, mental, spiritual, and emotional impacts of stress and anxiety. Dr. Cooke has also dedicated her time to conducting qualitative research on health disparities, and recently published an article entitled “Black women social workers: Workplace stress experiences.” Dr. Cooke enjoys painting and traveling in her spare time.

Stacy Kaminski Headshot

Stacy Kaminski

Operations & Systems Management Lead

Stacy Kaminski is the operations and systems management lead at EmblemHealth Health@Work. Under the direction of the medical director, she manages clinical operations including administration of the electronic medical records system, finances, and grant administration. She works closely with the clinical and administrative arms to ensure optimal service to the clinic’s patients, examinees, and clients. Stacy is a graduate of SUNY Albany, where she played for the tennis team and received a Bachelor of Science in English and economics. Stacy has years of experience in the administrative medical field, having worked in both private practice and hospital settings. During her free time, she enjoys restoring furniture, gaming, hiking, and kayaking.
Jeanvive Washington Headshot

Jeanvive Washington

Medical Specialist

Jeanvive Washington is the medical specialist at EmblemHealth Health@Work. Her role is to perform routine screening tests, including electrocardiograms, phlebotomy, height and weight measurements, and vitals checks. As the medical specialist, she also assists the doctor during examinations, collects and prepares laboratory specimens, and performs basic laboratory tests. Using the electronic medical records system and other computer applications, she ensures that all related reports, labs, and other examination paperwork are completed and available in the individual’s medical record before the appointment. Jeanvive has worked in the medical field for over 10 years. At the start of her health care career, she had an administrative role and assisted the elderly with their day-to-day duties. She enjoys spending time with her children, going out to eat, and traveling.

Sakina Boyd

Office Manager

Sakina Boyd is the medical office manager for EmblemHealth Health@Work. Sakina handles the clinic’s daily operations, including personnel management and ensuring policies and procedures are followed. She directly supervises the administrative arm of the clinic (including the medical receptionists), ensuring optimal clinic administrative performance. Along with the medical director and operations lead, she ensures that all staff members are adequately trained on clinic protocols and procedures. She provides clinical support as needed and administrative support to the director and medical staff within the department. Sakina also provides technical support, including ensuring quality of data within the electronic medical records system. She serves as the medical director’s primary point of contact for both internal and external interactions and works closely with other administrative staff to meet the administrative needs of the clinic. Sakina is a graduate of Albany High School and has her associate degree in medical assistance and criminal justice. She has years of experience in the administrative medical field, having worked in both private practice and hospital settings, and was a long-term administrative assistant for New York State. Sakina enjoys traveling and shopping during her leisure time.

Lekiya Dancy-Blalark

Registered Nurse

Lekiya Dancy-Blalark is the registered nurse at EmblemHealth Health@Work. In her role, she provides general occupational and environmental medical care. Her duties include occupational medicine preventive services, examinee evaluations, and patient support with work-related injuries/illnesses. Lekiya provides occupational health care and psychosocial supports for the clinic’s patients/examinees, and works closely with the other licensed health care professionals (including the social worker/case manager) for the benefit of clinic patients. She also delivers ancillary medical services that include, but are not limited to: testing required by OSHA-mandated evaluations, vaccinations, and other screening requirements (EKGs, pulmonary function screening, audiometry, and respirator fit testing). Lekiya helps provide nursing care and health care instructions to industries, other community groups, and patients. She supports workers’ health and safety by providing clinically relevant information and provision of safety and health trainings to various industries, other health professionals, and community-based organizations in response to grant requirements and other required educational services. Lekiya is a graduate of Maria College, class of 2020 and 2022. She holds an associate’s and bachelor’s degree in nursing and a NYS professional registered nursing license. Aside from working at EmblemHealth Health@Work, Lekiya loves traveling and spending time with her children, family, and friends.

Adam Black

Industrial Hygienist

Adam Black is the industrial hygienist at EmblemHealth Health@Work. His primary activities are respirator fit testing, workplace health and safety training, and customer support. Adam has 25 years of experience performing exposure monitoring, hearing conservation, injury investigation, ergonomic assessments, workplace regulatory compliance, workplace safety and compliance assessments/auditing. He has also managed a variety of written health and safety programs. Adam is a Certified Industrial Hygienist (CIH) and Certified Safety Professional (CSP). He attended Purdue University, and enjoys travel and food.
