The Social Worker/Case Manager provides support to patients that includes crisis intervention, patient education surrounding occupational health, counseling, advocacy, follow-up and, case management.
The Social Work program provides services that will help stabilize persons with work-related injury or illness and provide support as they navigate not only physical issues but also the challenges associated with their condition.
The Social Worker/Case Manager supports patients in exploring their work and career options and assists them in connecting with Vocational Rehabilitation programs, if necessary. This program provides case management for our patients, assisting them with various applications, referring them to community resources (e.g., Psychological Services, Food Pantries, etc.) and advocating for them to receive services (e.g., SNAP benefits, Medicaid/Medicare, etc.).
